Distance sets the floor, but it rarely sets the final number. Aircraft type, crew composition, oxygen and ventilator requirements, ground ambulance legs at both ends, departure timing, and destination airport access all move the figure. A patient who needs a respiratory therapist and a ventilator prices differently from a patient who can travel seated with a nurse escort on a scheduled airline.
